Insurance commissioner to charge $50 for filing of articles of incorporation, copies or for amendments, as well as $50 for the filing of articles of mergers or copies thereof.
Increases commissioner fees to $50 charge for filing of annual statement for a county mutual, $100 charge for the filing of an annual statement for a life settlement provider.
$50 for filing abstract of annual statement for any insurance company for publication.
Charge of $25 for each insurance company appointment, renewal of insurance provider
A $500 charge for initial application for multiple employer welfare arrangements, $100 for annual renewal; $100 for initial application for life settlement provider, $25 renewal
Life settlement broker application, an initial broker license fee of $100, $25 for renewal
Also $100 charge for issuing public adjuster license, $25 for each biennial renewal; $250 charge for issuing life or health insurance administrator license, $100 for renewal
If an amount of fee, penalty or interest has been paid which was not due, refund may be issued, must present claim for refund to commissioner within 2 years of payment.
